At CareOne, we encourage pre-admission visits prior to selecting a Nursing and Rehabilitation Center (also known as sub-acute care) so that your final decision is as sound as possible. If you are responsible for choosing a rehabilitation or Long Term Care Center for your family member, or if you are anticipating upcoming post operative rehabilitation for yourself, the benefits of a site visit are extremely valuable.

The Benefits of the pre-admission visit to our center include:

• Meeting Key Clinical care staff, including physical, occupational and speech therapists, nursing team, dietician, social worker, and administrative team who will be overseeing your loved one’s care plan.

• A tour of the rehabilitation patient unit and therapy gym, including a full tour of patient rooms. Based upon availability, you may also have the opportunity to select the exact room your loved one will be staying in.

• Obtain answers to important questions from our clinicians such as the general course of clinical treatment, and answers to questions about the clinical services offered in our center.

• Learn more about Medicare and other insurance coverage and related issues so that you have a comprehensive understanding of financial related issues.

• Talk to current patients regarding their satisfaction and experience at our center.

• Better understanding to the transition process from the hospital to rehabilitation, what is involved, what is covered under insurance, and how the CareOne staff will assist you and your family member every step of the way.

Our Center is open for tours seven days a week from 8:00 a.m. to 8:00 p.m.
